To expedite time at our office, please print and complete our registration forms and bring them to your appointment. To do this, please click on the first page (CCC Form1.bmp). Once the photo slideshow opens, you can right-click the image and click "Save As". Once you have saved the image to your computer, you can print it out. This must be done for all eight (8) pages of the CCC form. Thank you.

Also, after you have completed filling out the forms in pen. You can scan and e-mail the completed forms to revrichard@bellsouth.net
